
The transition to adult services can be a daunting process for patients and parents. We created this Self-Advocacy Pack to empower our young adult members with the knowledge and tools needed to effectively manage their healthcare, communicate their needs, and navigate adult healthcare services:
We are aware that transition from specialised care in a children’s and/or adolescent clinic to local care in an adult clinic can be a daunting process for patients and parents. An article written last year by one of the Caring Matters Now Young Ambassadors described some of the challenges and made recommendations for supporting this transition. In response to these recommendations and with the help of our Research Connector Volunteer, we created this self-advocacy pack to empower you with knowledge and tools to effectively manage your health and communicate your needs as you navigate the transition to adult healthcare services.
